Inspector Notes

The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on December 12, 2023 regarding allegations of special care provisions and emergencies. The inspection was initiated on December 12, 2023 at approximately 9:45 AM and concluded on December 12, 2023. Interviews with staff were completed, observations were made in the center, and documentation was reviewed.

The preponderance of the evidence gathered during the investigation does not supports the allegation; therefore, the complaint is determined to be not valid. Any violation(s) not related to the complaint but identified during the investigation can also be found on the violation notice.

Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names, list staff by positions only.

Based on review of children records, interview, and observation, the center failed to operate within the terms of their current license issued by the department.

Evidence:
1. The center is currently licensed for children ages 3 years to 5 years, 11 months. The record for child #2 (enrolled: 9/25/23) was 2 years at time of enrollment. The record for child #5 (enrolled: 9/05/23) was 2 years, 7 months at the time of enrollment.
2. Administrator verified that child #2 and child #5 were not three years of age at time of enrollment.
Plan of Correction: Modification request has been submitted (1/11/24). Center will ensure not to accept 2 years until license modification is approved.
