Inspector Notes
An unannounced renewal inspection was conducted today from approximately 11:40am-1:40pm. There were 64 children in the direct care and supervision of 14 staff. A sample of 5 children?s records, and 5 staff records were reviewed. The children were observed napping. Zero new staff have been hired since the last inspection. Evidence:
Disinfectant spray was located in an unlocked cabinet in the Toddler room.
Evidence:
Emergency phone numbers were not posted near the telephone located in the office.
Evidence:
5 of 5 children's records reviewed were missing required information.
1) The records of Children A through D were missing a start date.
2) The records of Children B and D were missing the phone number of their parents employer.
3) The record of Child E was missing proof of identity.
