Inspector Notes
An unannounced inspection was initiated on 5/29/2024 and concluded on 5/29/2024 in response to a self-report received by the licensing office on 5/24/2024 relating to allegations of forbidden actions. The inspector reviewed two child records and three staff records on-site. This inspection included document review and interviews.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

The center failed to ensure that physical punishment, striking a child, roughly handling or shaking a child, restricting movement through binding or tying, forcing a child to assume an uncomfortable position, or exercise as punishment does not occur.
Evidence: Based on staff interview, Staff #1 admitted to spanking Child #1.

Plan of Correction: Administration removed the staff member from the classroom. Procedures and policies were followed for an internal investigation; resulting in dismissal from employment. Proper agencies were also notified of this incident through self-reporting. Staff will be reminded of and retrained on appropriate actions and guidance of children.
