Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 8/16/2024 and completed on 8/16/2024. The on-site inspection began at 9:45 am and ended at 12:00 pm. The inspector reviewed compliance in the areas listed above. There were seven children present and six staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review, the center failed to ensure injury prevention procedures be updated at least annually based on documentation of injuries and a review of the activities and services.

Evidence:
The last updated documented injury prevention procedure was dated 2021.
Plan of Correction: The Director will update injury prevention procedures and ensure yearly updates.
