During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on and completed on 2/26/2025. The on-site inspection began at 9:00 am and ended at 11:00 am. The inspector reviewed compliance in the areas listed above. There were 15 children present and six staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Electrical outlets shall have protective covers that are of a size that cannot be swallowed by children.

The center had one classroom that did not have a protective cover over an electrical outlet.
Plan of Correction: The Director placed a protective cover over the electrical outlet.
Standard 8VAC20-780-40-M
The center is required to keep a current list of all known allergies, sensitivities and dietary restrictions in each room or area where children are present. The list must be dated and confidential.

The allergy lists in the center were not confidential.
Plan of Correction: The Director will make all posted allergy lists confidential.
Standard 8VAC20-780-570-A
High chairs, infant carrier seats, or feeding tables shall be used for children under 12 months who are not held while being fed.

An infant, under 12 months old, was propped up by a pillow in the floor while feeding.
Plan of Correction: The Director will retrain staff that infants under 12 months old must be in a high chair, infant carrier or feeding table while feeding.

Prepared infant formula shall be refrigerated, dated and labeled with the child's name.

An infant's bottle was not labeled with the child's name and was not dated.
Plan of Correction: The Director will ensure staff working in the infant room will label bottles with the date and child's name.
