During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 03/06/2025, as a part the licensure period, The on-site inspection began at 11:25 am and ended at 1:15 pm. The inspector reviewed compliance in the areas listed above. There were 42 children present and 11 staff. The inspector reviewed 3 children?s records and 2 staff records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Each staff member shall submit documentation of a negative tuberculosis screening completes within the last 30 calendar days of the date of employment.

Staff #1 Tb screen was completed approximately 3 months prior to hire date.
Staff #2 TB screen was completed 12 days after date of hire.
Plan of Correction: We will make sure that TB test are completed before hire date.
Standard 8VAC20-780-270-A
Areas of the center were not maintained in a clean condition.
There were rust stains on the floor of the blue room where the cots are stored and on the bottom panel of the brown cabinet.
Plan of Correction: We will have the floor cleaned and cover the rust on the brown cabinet.
