During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was completed on 7/1/2025, as a part of the licensure period. The on-site inspection began at 8:25 am and ended at 10:45 am. The inspector reviewed compliance in the areas listed above. There were 41 children present and 9 staff. The inspector reviewed 3 children?s records and 3 staff records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 22.1-289.035-B-1
The center is required to provide a sworn statement or affirmation disclosing whether he has ever been convicted of or is the subject of pending charges for any offense within or outside the Commonwealth and whether he has been the subject of a founded complaint of child abuse or neglect within or outside the Commonwealth prior to the employee's first day of employment.
Staff A was employed 5 days when the sworn disclosure statement was signed.
Plan of Correction: We saw that the sworn statement was not signed by first day, as soon as we caught it during orientation training we had it signed.
Standard 8VAC20-780-160-A
Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening and shall be submitted at the time of employment and completed within last 30 calendar days of date of employment.
Staff A provided a Tuberculosis screen by first date of employment, however it was dated 7 months prior to hire date.
Plan of Correction: Tb screen was completed prior to hire date. Doctor did not redate the screen. I will go today to have them correct.
Standard 8VAC20-780-40-M
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present.
The center did not have a list of children's allergies in each classroom. There were action plans with child information and picture.
Plan of Correction: We will put a cover on the lists.
Standard 8VAC20-780-60-A
The center shall maintain a record for each child in care with documentation of annual up to date information .
Child #1 annual update was over due 8 months.
Child #2 annual update was over due 1o months.
Plan of Correction: We will create a process to review all children's files.
Standard 8VAC20-780-70
The center shall document two or more references as to character and reputation as well as competency before employment.
Staff A was employed one week when references were documented in record.
Plan of Correction: Reviewed with program administrators will follow up. Will make sure they are done before start date while onboarding.
