Inspector Notes

An unannounced, on-site monitoring inspection was initiated and concluded on May 2, 2024 from 3:15 p.m. to 4:15 p.m. The inspector reviewed compliance in the areas listed above. There were 11 children present and 2 staff. The inspector reviewed 5 children?s records and 1 staff record on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The facility failed to have required information in each child's file. One child did not have documentation of the name, address, and phone number of two designated people to contact in an emergency if a parent cannot be reached.
Plan of Correction: Center staff will request the information from the parent.
