Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 12/06/2024 and the inspection concluded on 12/11/2024. The on-site inspection began at 3:15 p.m. and ended at 4:30 p.m. The inspector reviewed compliance in the areas listed above. There were 11 children present and 2 staff. The inspector reviewed 6 children?s records on site and 2 staff records electronically on 12/11/2024. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-560-F
When centers choose to provide meals or snacks, the menu shall be posted in a place conspicuous to parents or given to parents. The menu posted on December 6, 2024 was for November.
Plan of Correction: The December menu will be posted.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which contains required information. Child 6, enrolled for over one year, did not have
documentation of the name, address and number of two emergency contacts.
Plan of Correction: Center director will request two emergency contacts from the parent.
