Inspector Notes

An in-person renewal inspection was conducted on January 9. 2023. There were 2 children in care (6 pts) with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Children were observed playing freely, singing songs, and dancing. A total of 2 child records, 2 staff records and 3 household member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the provider did not obtain documentation of a physical examination within the first 30 days of attendance.

Evidence: Child 1 and 2 did not have documentation of physical exam on record. (Child 1, start 9/22; Child 2, start 8/22)
Plan of Correction: We will contact the parents for the physical examination documentation.

Based on observation, the first aid kit did not have all required items.

Evidence: The first aid kit is missing two triangular bandages.
