During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on December 30, 2024 and completed on December 30, 2024. The on-site inspection began at 9:15am and ended at 10:30am. The inspector reviewed compliance in the areas listed above. There were 5 children present and 2 caregivers; the point total was 14. The inspector reviewed 4 children?s records and 4 caregiver/household member(s) records on-site December 30, 2024. This inspection included document review, tour of the facility, and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider will obtain documentation of a physical examination prior to a child's attendance or within 30 days of first day of attendance.

There was no documentation of a physical examination in Child A, B and C's record prior to attendance or within 30 days of the first day of attendance.
Plan of Correction: I will notify the parents for all three children to supply the physical examination.
Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and adult household member a current Tuberculosis screening form every two years.

The provider did not have a current Tuberculosis screening form for Staff A, B and C. The last documented screening was more than two years prior.
Plan of Correction: I will get the three current Tuberculosis screenings as required.
Standard 8VAC20-800-780-B
The Provider shall review with the parent the emergency contact information to ensure it is correct.

Child A and C were missing the annual review of emergency contact information documentation.
Plan of Correction: I will get the parents to review the emergency contact information this evening at pick up.
Standard 8VAC20-800-90-C
The provider shall obtain documentation of additional immunizations every six months for a child under the age of two years.

Child A had no current immunization documentation record. The last immunization documentation was August of 2023.
Plan of Correction: I will get the current immunization documentation from the parent for Child A.
