During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on April 15, 2025 and completed on April 16, 2025. The on-site inspection began at 12:25pm and ended at 2:00pm. The inspector reviewed compliance in the areas listed above. There were nine children present and two caregivers; the point total was 25. The inspector reviewed four children?s records, three caregiver records, and two household members records on-site and electronically on April 16, 2025.

This inspection included document review, a tour of the family day home, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

There were no physical exam records available for Child #1, who had been attending for one month and three weeks, and for Child #4, who had been attending for two years.
Plan of Correction: The provider will request copies of completed physical records from parents.
Standard 8VAC20-800-170-A
Providers must obtain a current tuberculosis screening from each adult household member prior to coming into contact with children.

There was no tuberculosis screening for Household Member A available to verify that a screening has been completed within the past two years.
Plan of Correction: The provider will obtain a completed TB screening for the household member.
Standard 8VAC20-800-780-B
Annually, the provider shall review with the parent the emergency contact information required in each child?s record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review.

The record for Child #4 did not have a parent?s signature for an annual review of the emergency contact information. Child #4 had been in care for two years.
Plan of Correction: The provider obtained documentation of parent review of child's emergency contact information.
