Children's records are required to contain written authorization for emergency medical care. The record for Child #3 was missing the written authorization for emergency medical care. The child was present during the inspection and had been in care for a month.

Standard 8VAC20-800-780-B
Annually, the provider shall review with the parent the emergency contact information required in each child?s record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review. Records for children #1, #2, #4 and #5 did not have a parent?s signature for an annual review of the emergency contact information. Child #2 and #5 had been in care for over a year. Children #1 and #4 had been in care for over two years without any annual review.
