Inspector Notes

A subsidy health and safety (SHSI) inspection was completed on 08/10/2023. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. There were no children present in care at the time of inspection on 8/10/2023.
Information gathered during the inspection determined violations with applicable standards or law. Violations are identified on the violation notice.

Standard 8VAC20-790-190-B
Based on a record review and interview, the vendor did not ensure that each enrolled child's record contains all of the required information to include the name, address and phone number for both custodial parents home and workplace and the name address and phone number of at least one emergency contact.

Evidence:
Four children's records were reviewed.

The record for child #2 and child #4 contained no information regarding the children's fathers' names, the fathers' home address and phone number nor the fathers' place of employment address and phone number or any indication of non-applicability. Child #1's and Child #4's farther are listed on the birth certificate copies in child #2 and child #4's records.

The records for child #1 and child #2 did not contain an address for the listed emergency contact to be called in the case of an emergency if the parent's can not be reached.
Plan of Correction: The vendor stated that she would ask the parents of child #1, #2, #3 and #4 to complete the missing information in their record and/or write Not applicable or unknown as appropriate by Monday.
All parent s will be asked to leave no blank spaces on the applications but to fill in the requested information or indicate that it is unknown or not applicable.
Standard 8VAC20-790-250-F
Based on a review of records and interview, the vendor has not annually attended the department's health and safety update course.
Evidence:
The last documented training for the Departments Virginia Health and safety training in the vendor's file was dated 7/28/2022 which is greater than 12 months ago.
The vendor confirmed that she had not taken the annual update training in the last 12 months.
Plan of Correction: The vendor stated that she would complete the annual health and safety training update by Monday 8/14/2023.
