Inspector Notes

A renewal inspection was conducted on January 4, 2023. There were twenty-eight children present, ranging in ages from two years to four years, with four staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and two staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review, facility did not ensure prescription and nonprescription medication is given to a child only with written authorization from the parent.

Evidence: According to the medication log, a nonprescription medication was given to child #2 on 12-7-2022 and there was not written authorization from the parent to administer.
Plan of Correction: Medication will be returned to parent.
Standard 8VAC20-780-550-P
Based on review, facility did not ensure required information is documented when a child sustains an injury while in the program.

Evidence: An injury report completed on 12-21-2022 for child #3 did not document the date and time parent was notified of the injury.
Plan of Correction: Information will be added to the injury report. Staff will be reminded to fill out form completely.
