Inspector Notes
A renewal inspection was conducted on February 23, 2023 from 8:50 A.M.-11:30 A.M. There were seven children present, ranging in ages from three years to five years of age, with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and six staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

3Evidence:
1. Child #1's immunizations were dated 11/10/2022. First day of attendance for Child #1 was 11/09/2022.
2. Child #3's immunizations were dated 02/07/2023. First day of attendance for Child #3 was 01/12/2023.
3. Child #5's immunizations were dated 02/07/2023. First day of attendance for Child #5 was 01/30/2023.
4. Staff #1 verified the dates of enrollment and the dates on the immunization records.
Evidence:
1. Staff #2's orientation training was dated 02/08/2023. Date of hire for Staff #2 was 09/26/2022.
2. Staff #3's orientation training was dated 02/18/2023. Date of hire for Staff #3 was 09/26/2022.
3. Staff #1 verified the date of hire for Staff #2 and #3 and the dates on the orientation certificates.
Evidence:
1. In the Pre-K Classroom after the children finished snack they did not wash their hands with soap and water or a disposable wipe. The children left the snack table and went to the rug area and got a book to look at.
2. Staff # 4 verified that the children did not wash their hands with soap and water or a disposable wipe after snack.
