During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on September 16, 2024 and completed on September 20, 2024 as a part of an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as result of the last inspection related to Background Checks and Special Care Provisions and Emergencies. The on-site inspection began at 4:05 p.m. and ended at 5:15 p.m. The inspector reviewed compliance in the areas listed above. There were 18 children present and 2 staff. The inspector reviewed three children?s records and three staff records on-site. This inspection included document review, tour of the facility and interviews

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center is required to obtain background checks from any state or U.S. territory in which the individual has resided in the preceding five years.

Staff #1, employed for more than three weeks, did not have documentation of requesting a central registry check, sex offender registry check from one territory
Plan of Correction: Staff #5 does have a first aid certification, but we do not have a copy of it. We have requested a new copy.
