Inspector Notes

An unannounced on-site monitoring inspection was conducted on 11/16/23 from 2:30pm until 3:15pm. At the time of entrance, there were 43 children in care with 3 staff present. Children were observed eating snack, completing homework and engaging in free play. Restroom and handwashing procedures and parent pick-up procedures were also observed. A sample of 5 children's records were reviewed. First aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills, children's injury reports, medication and required center postings were also reviewed.
A sample of 4 staff records were reviewed at the Greenbrier North YMCA on 11/16/23 from 10:15am until 11:00am.
Information gathered during the inspection determined non-compliances with applicable standards or law. One violation is documented on the violation notice issued to the program and discussed with center administration at the conclusion of the inspection.

Based on record review and interview, the center did not ensure that the results of a fingerprint based background check are obtained at least every five years for all staff.

Evidence:
1. The most recent fingerprint based background check for staff 1 is dated 9/20/18. Staff 1 was due for an updated fingerprint based background check in September 2023.
2. Center administration confirmed that the fingerprint based background check for staff 1 is expired.
Plan of Correction: The center responded with the following: Staff 1 has an appointment to submit his fingerprints for an updated background check on 11/18/23. Once obtained, the results will be forwarded to the licensing inspector. Going forward, center administration will ensure updated background checks are obtained within the required time frames.
