During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced , on-site monitoring inspection was initiated on 12/06/2024 and completed on 12/06/2024. The on-site inspection began at 4:30 pm and ended at 5:31 pm. The inspector reviewed compliance in the areas listed above. There were 30 children present and 3 staff. The inspector reviewed 6 children's records on-site. On 12/11/2024 the inspector reviewed 3 staff records on-site at the Greenbrier North YMCA where staff records are maintained.

Standard 8VAC20-780-160-A
The documentation of the tuberculosis screening shall be submitted at the time of employment and shall have been completed within the last 30 calendar days of the date of employment.

The tuberculosis screening for staff 1 was completed more than 30 days after the first date of employment.
Plan of Correction: Administrative staff stated that going forward - all new staff will obtain a TB screening prior to hire and will ensure completion within 30 days prior to hire.
Standard 8VAC20-780-340-F
Children under 10 years of age always shall be within actual sight and sound supervision of staff, except that staff need only be able to hear a child who is using the restroom.

1. During the on-site inspection on 12/06/2024 the inspector observed child 1 and child 2 return from the bathroom at approximately 4:35 pm. The location of the bathroom is not within sight and sound supervision of the cafeteria where the after school program is located.
2. The door to the cafeteria was closed and no staff were in the hallway to monitor child 1 and child 2 while they were in the bathroom.
Plan of Correction: The staff on-site stated it is the usual practice for staff to escort children to the bathroom. Going forward all children will be escorted to the bathroom and/or a staff member will stand in the hallway to monitor children.
