Inspector Notes
An unannounced monitoring inspection was initiated on 10/23/2025 and completed on 11/19/2025 in response to a self-report received by the licensing office on 10/21/2025 relating to supervision and special care provisions and emergencies. The inspector reviewed 1 child's record on-site on 10/23/2025 and 1 staff record on-site on 11/19/2025. 1 staff record was reviewed electronically on 11/20/2025. This inspection included:

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found in administration and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/04/2025. A POC submitted after this date will not appear on the public website.
Violations
51. Child 1 fell from the monkey bars of the outdoor play structure on 10/17/2025 at approximately 3:35 pm. Child 1 landed on both hands and told staff 1 that her hands hurt. Staff 1 gave child 1 an ice pack but provided no further monitoring or follow-up.
2. Staff 1 did not notify any of the staff present that child 1 had fallen off the play structure.
3. The parent of child 1 stated when she arrived at 4:49 pm to pick-up her child she was holding her wrist and was "tearful and sad." Child 1 told her parent that her wrist hurt.
4. Child 1 required medical attention with a diagnosis of a buckle fracture to both wrists.
1. Staff 1 who was supervising outdoor play on 10/17/2025, stated she did not see child 1 fall from the monkey bars of the outdoor play structure. Child 1 sustained an injury that required medical attention.
a. Staff 1 stated she was not aware that child 1 had fallen from the play structure until the child came to her crying.
b. Staff 2 stated she did not see child 1 fall as she was positioned on the opposite side of the playground.
1. The written playground safety plan provided to the inspector did not include provision for active supervision by staff to include positioning of staff in strategic locations, scanning play activities, and circulation among children.
2. Staff 1 and staff 2 stated this was the safety plan they had received training for when supervising children on the playground.
The facility did not notify the parent of child 1 of a fall that occurred during outdoor play on 10/17/2025 at approximately 3:45 pm.
Staff did not write an injury report on the day of occurrence for an injury received by child 1 on 10/17/2025.
