The center shall inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.
A serious injury requiring outside medical attention that occurred on 11/03/2025 was not reported to the department until 11/14/2025. Staff 1 stated that the parent of child 1 notified them that the injury required medical attention on 11/04/2025.

Plan of Correction: Went over proper procedures with the staff on 12/18/2025 when incidents happen at the site. Will ensure proper incident reporting procedures are followed.
