Inspector Notes
An unannounced renewal inspection was conducted on March 23, 2023. The inspector was on site from approximately 10:30 am until 12:00 pm. The director was present and assisted with the inspection. There were three children present, ranging in ages from two years to three years, with two staff
supervising. Children were observed playing in centers, and coloring. Later children prepared for lunch with handwashing. Nap time was also observed. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and four staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have questions regarding this inspection, please contact the licensing inspector.

The record of staff #4, employed 03/06/2023, contained a sworn statement dated 03/23/2023, exceeding the first day of employment.
Evidence:
The most recent tuberculosis screening for staff #3 was dated 06/12/2019 exceeding two years. The director stated that an updated tuberculosis TB screening had not been completed.
Evidence:
The record of staff #4, employed 03/06/2023, did not contain documentation of orientation training. The director stated, orientation had not been completed as of today, March 23, 2023, exceeding seven days from employment.
Evidence:
The most recent health inspection for the facility was dated 10/26/2021, exceeding the annual requirement.

The first aid kit had only one tri-angular bandage when at least two are required
