Inspector Notes

An unannounced renewal inspection was conducted on March 23, 2023. The inspector was on site from approximately 10:30 am until 12:00 pm. The director was present and assisted with the inspection. There were three children present, ranging in ages from two years to three years, with two staff
supervising. Children were observed playing in centers, and coloring. Later children prepared for lunch with handwashing. Nap time was also observed. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and four staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have questions regarding this inspection, please contact the licensing inspector.

Standard 8VAC20-770-60-B
Based on review of four staff records, the facility did not obtain a completed sworn statement prior to the first day of employment for one staff member.

Evidence:

The record of staff #4, employed 03/06/2023, contained a sworn statement dated 03/23/2023, exceeding the first day of employment.
Plan of Correction: Corrected on March 23, 2023. Per the director: Going forward we will ensure staff complete the sworn statement at or before the first day of hire.
Standard 8VAC20-780-160-C
Based on review of four staff records and interview, the facility did not obtain the results of a follow up tuberculosis screening at least every two years from the date of the initial screening for one staff member.

Evidence:

The most recent tuberculosis screening for staff #3 was dated 06/12/2019 exceeding two years. The director stated that an updated tuberculosis TB screening had not been completed.
Plan of Correction: Per the director: We will have the staff obtain an updated TB screening tomorrow.

Based on review of four staff records and interview, the facility did not ensure one staff completed orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.

Evidence:

The record of staff #4, employed 03/06/2023, did not contain documentation of orientation training. The director stated, orientation had not been completed as of today, March 23, 2023, exceeding seven days from employment.
Plan of Correction: Per the director: I will finish the orientation training with the staff today.

Based on review of documentation, the facility did not obtain annual approval from the health department.

Evidence:

The most recent health inspection for the facility was dated 10/26/2021, exceeding the annual requirement.
Plan of Correction: Per the director: We will contact the health department and request they conduct the inspection.
