Inspector Notes

A renewal inspection was conducted on 2/22/2023 with the center director and staff. There were 30 children present, ranging in ages from 5 years to 12 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 1 staff record were reviewed. The children were playing outdoors and working on a STEAM project.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of 5 prescription medications, the facility failed to have written authorization
from the parent for the program to administer medication. Evidence: Child A and Child B did
not have medication authorizations that authorized the program to administer medication.
Plan of Correction: Center staff will obtain written parental authorization to administer medication.
