Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 5/07/2024 and the inspection concluded on 5/21/2024. The on-site inspection began at 4:30 p.m. and ended at 5:45 p.m. The inspector reviewed compliance in the areas listed above. There were 22 children present and 3 staff. The inspector reviewed 6 children?s records on site and 3 staff records electronically on 5/16/2024. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Documentation of a physical examination must be submitted for each child within 30 days after the first day of attendance. Child 4 did not have documentation of a physical on file.
Plan of Correction: Center staff will request the physical from the parent.
