During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on March 5, 2025 and the inspection concluded on March 14, 2025. The on-site inspection began at 4:00 p.m. and ended at 5:30 p.m. The inspector reviewed compliance in the areas listed above. There were 39 children present and 4 staff. The inspector reviewed 5 children?s records on-site and 1 staff record electronically on March 7, 2025. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-245-J
Staff members caring for any child for whom emergency medications have been prescribed must have the appropriate Medication Administration Training. There were four emergency medications on site, but none of the staff on site had Medication Administration Training . One of the medications requires Medication Administration Training - Seizure Medications; there were no staff on site certified in this training.
Plan of Correction: Someone with MAT training will always be on site when children with medications are present,
Standard 8VAC20-780-340-G
Children 10 years of age and older shall be within actual sight and sound supervision of staff except when there is a system to ensure that individuals who are not staff members or
persons allowed to pick up children in care do not enter the areas where children are not under sight supervision, A 10 year old child was out of sight and sound supervision when going to a bathroom that was not adjacent to the cafeteria where staff were located. Staff did not ensure that unauthorized adults were not present.
Plan of Correction: Children will be in sight and sound supervision per center policy.
Standard 8VAC20-780-510-E
The center shall ensure that procedures for administering medication shall include methods to prevent use of outdated medication. The emergency medication for one child expired
2/28/2025 and the parent had not been notified that a replacement was needed.
Plan of Correction: A current medication will be requested from the parent.
Standard 8VAC20-780-530-A-1
At least one staff in each grouping of children shall have current cardiopulmonary resuscitation (CPR). Staff 1 was alone with 15 school age children, but does not have documentation of current CPR on file.
Plan of Correction: Staff will not be alone with children until first aid and CPR certification is obtained.
