Inspector Notes

An unannounced, on-site monitoring inspection was initiated on April 29, 2024 and concluded on May 5, 2024. The on-site inspection began at 3:30 p.m. and ended at 5:00 p.m. The inspector reviewed compliance in the areas listed above. There were 10 children present and 2 staff. The inspector reviewed 6 children?s records on site and 1 new staff record electronically on May 3. 2024. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The facility failed to have required information in each child's file. Two of six children did not have documentation of the name, address, and phone number of two (non-parent) designated people to contact in an emergency if a parent cannot be reached.
Plan of Correction: Staff will request 2 emergency contacts for the two children.
