Inspector Notes
An unannounced monitoring inspection was conducted on Wednesday, February 15, 2023, to determine the center's compliance with licensing standards. The inspection began at 3:40pm and concluded at 5:10pm. The center's director was available throughout the inspection. The census for today's inspection consisted of 14 children in care and 3 staff supervising. During the tour, the inspector observed the children eating dinner, completing homework, and interacting with peers and staff. The inspector inspected the inside and outside play area. Staff was observed interacting with children in a positive manner and the inspector interviewed staff and children when time allowed. The required postings were inspected and found to be in compliance. Liability insurance, health inspection, and drills were all in compliance. There were five children's and 3 staff records reviewed. Staff records were reviewed remotely. There were violations cited during this inspection.

1. In Staff#1's record (employed11/29/2022), there was no results of an out of state central registry search found for review.
2. Interview with staff confirmed a search of the out of state central registry for Staff#1 was requested 11/11/2022 but had not received the results.
Evidence:
1. The menu for dinner listed corndog, mixed fruit, french fries, and milk. The children were observed to be served oranges, corn dog, french fries, and milk. The menu was not changed to reflect the substitution for the fruit.
