Inspector Notes

A monitoring inspection was conducted on October 24, 2023 with the center director and staff. There were 75 school age children present, with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 8 child records and 3 staff records were reviewed. The children were making seasonal art activities and playing outdoors. Staff files were submitted electronically on 10/31/2023.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of three staff records, the center failed to ensure that out of state background checks were conducted as required. Evidence:
1. Staff 2 (date of employment 8/15/2023) indicated on the sworn disclosure statement dated 8/11/2023 that they had lived outside the state of Virginia within the past five years. There was no documentation to support that the required search of the child abuse and neglect registry was submitted within 30 days as required. (Kentucky). Records showed that two criminal record checks were conducted and the sex offender registry had been checked, but the out of state search of the child abuse and neglect registry (CPS check) was not done.
Plan of Correction: Administration will ensure that out of state record checks are conducted as required. The facility thought a criminal record check and search of the central registry were conducted, but two criminal record checks were done.
