Inspector Notes

An announced initial inspection was conducted on 07/26/2022. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, emergencies and special services. A total of 5 board officer/agent records were reviewed. The center was measured to determine the license capacity and policy and procedures were reviewed. The inspector discussed the following with the provider: policy and procedures, emergency preparedness plan and emergency supplies. The inspector arrived for the inspection at 10:00 am and departed at 1:30 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review and discussion with staff, the center failed to ensure all applicants for licensure obtain a child abuse and neglect search and a criminal record search from any state in which the individual has resided in the preceding five years.
Evidence: The record for board officer/agent 2 indicated board officer/agent 2 has resided outside of the state of Virginia within the preceding five years. The sworn disclosure statement for board officer/agent 2 was dated 3/23/2021. There was not a child abuse and neglect search nor criminal record search from all the states the individual had resided in during the preceding five years available for review during the inspection.
