Inspector Notes

An unannounced renewal inspection was conducted on 01/12/2023. There were 48 children, ages 5 years- 11 years, and 8 staff members providing direct supervision present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, special care provisions and emergencies and special services. The children were observed during mealtime, individual activities, and large motor activities. The inspector discussed the following with the provider: medication. A total of 5 children?s records, and 3 medications were reviewed. The staff records are kept off site at the corporate office and will be reviewed at a later date. The inspector arrived for the inspection at 2:30 pm and departed at 3:45 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation noticed issued to the provider.

Based on record review and interviews with staff, the center failed to ensure each child shall have a physical examination by or under the direction of a physician before the child?s attendance or within 30 days after the first day of attendance.
Evidence: The record for Child 5 did not contain a physical examination. The record indicated the child?s first day of attendance was 05/31/2022. Staff 1 confirmed the documentation was not available.
