Inspector Notes

An unannounced complaint inspection was conducted on 12/18/2023 in response to allegations received by the licensing office on 12/11/2023 in the areas of supervision. The inspection concluded with an electronic record review on 12/21/2023. There were 33 children, ages 5 years -11 years, and 6 staff providing direct care and supervision to children present during the inspection. Staff were interviewed and a total of 4 children?s records and 5 staff records were reviewed. The inspector arrived for the inspection at approximately 2:45 pm and departed approximately 3:45 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

The complaint was determined to be valid.

Based on interviews and observation, the center failed to ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff.

Evidence:
1. Staff 2 confirms an incident occurred on 12/08/2023 where Child 1 was located by public school staff in restroom A while out of actual sight and sound supervision of program staff.
2. At approximately 3:02 pm on 12/18/2023, the inspector observed Child 3 and Child 4 leave restroom A and return to the cafeteria out of actual sight and sound supervision of program staff. Restroom A is approximately 85 feet from the cafeteria.
3. At approximately 3:03 pm on 12/18/2023, the inspector observed Child 2 leave the cafeteria, enter restroom B, and then return to the cafeteria out of actual sight and sound supervision of program staff. Restroom B is approximately 105 feet from the cafeteria.
Plan of Correction: Staff will ensure children are within actual sight and sound of staff.
