During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site renewal inspection was initiated on 01/23/2025 and completed on 01/28/2025. The on-site inspection began at 3:15 p.m. and ended at 4:05 p.m. The inspector reviewed compliance in the areas listed above. There were 28 children present and 6 staff. The inspector reviewed 5 children?s records on-site on 01/23/2025 and 5 staff records and 5 agent/applicant records on-site at the administration office on 01/28/2025. This inspection included document review, tour of the facility, interviews, and observations.

REPEAT VIOLATION
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

There was no documentation of immunizations in the record of Child 1, who had been enrolled for eleven months. There was no documentation of immunizations in the record of Child 2, who had been enrolled for over 3 years.
Plan of Correction: Staff will obtain.
Standard 8VAC20-780-140-A
REPEAT VIOLATION
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was no physical examination in the record of Child 1, who had been enrolled for eleven months. There was no physical examination in the record of Child 2, who had been enrolled for over 3 years.

The following non-medical emergency supplies shall be required: one working battery-operated radio in each building used by children.
