Inspector Notes
An investigation was initiated from 11:30 am - 2:00 pm in conjunction with a monitoring inspection in response to a self reported incident regarding supervision. Interviews were conducted, documentation, staff and children's records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on interviews it was determined that children did not always remain within sight and sound supervision of staff as required. Evidence: On 9/13/23 at around 5:00 pm two staff and eight children from the toddler class were lining up to come inside from the playground. Staff A was in the front of the line and staff B was in the back of the line. Staff and the children walked into the toddler classroom. Staff A kept two children in the toddler room while Staff B escorted the rest of the children to the activity room. A name to face attendance check was not conducted upon arrival to the toddler classroom or the activity room. Child A (20 months) remained outside for around 5-9 minutes. Child A's parent arrived at 5:09 pm to the activity room to retrieve their child. The parent was directed to the toddler room. As the parent was walking into the toddler room Staff A observed child A still playing on the playground and ran outside to retrieve child A. The child was then brought inside to their parent.
Plan of Correction: Review of sight and sound with all staff. Review of counting children and when to count. Review of lost child procedure.
