During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 08/13/2025, as a part of the provisional licensure period. The on-site inspection began at 2:15 p.m. and ended at 3:45 p.m. The inspector reviewed compliance in the areas listed above. There were 62 children present and 11 staff. The inspector reviewed four children?s records and two staff records on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/02/2025. A POC submitted after this date will not appear on the public website.

The center's procedures for administering medication shall include methods to prevent use of outdated medication.

The medication for child A has been expired for 13 days. Child B has two emergency medications on site. Medication one for child B has been expired for 43 days and medication two for child B has been expired for 13 days.
Plan of Correction: Sent expired meds home. Spreadsheet with list of meds in school with expiration dates. Obtained new meds for all expired meds.

REPEAT VIOLATION
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

There were eleven children present in the school age room, but ten children were marked present on the attendance record.
Plan of Correction: Training staff on software-based attendance. Monitoring attendance through out day.
