During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

Amended: An unannounced, on-site renewal inspection was initiated and completed on January 22, 2025. The on-site inspection began at 10:00am and ended at 1:00pm. The inspector reviewed compliance in the areas listed above. There were 65 children present and 13 staff. The inspector reviewed 6 children?s records and 8 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.036-A
The center is required to have board officers/agents undergo a background check every five years. The most recent criminal history and central registry for Staff #7 was overdue by a year and nine months.
Plan of Correction: Since he does not come to the building, we did not realize the background search had to be repeated. We will schedule an appointment for the background searches.
Standard 8VAC20-780-500-A
Children's hands shall be washed with soap and running water after toileting and any contact with blood, feces, or urine. Staff #2 changed a child's diaper in classroom #4 and did not wash the child's hands with soap and water.
Plan of Correction: The staff were reminded to wash children's hands after each diaper change.
Standard 8VAC20-780-500-B
Individual disposable barriers may be used between each diaper change. Staff #2 in Classroom #4 used a non-disposable barrier when changing a child's diaper.
Plan of Correction: The non-disposable barriers were removed and will not be used.
