Inspector Notes

An unannounced inspection was initiated on June 10, 2025 and completed on June 11, 2025 in response to a complaint received by the licensing office on June 4, 2025 relating to allegations of forbidden actions and behavior guidance. The inspector reviewed one child's record and five staff records on-site. This inspection included document review, tour of the facility, and interviews.

The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to administration and are documented on the violation notice issued to the program.

At least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department staff members shall obtain and submit the results of a follow-up tuberculosis (TB) screening.

The last documented TB screening for Staff #1 expired over ten months and the TB screening for Staff #5 expired six weeks ago.
Plan of Correction: Both staff members will go today for a TB screening.
