During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on September 8, 2025. The on-site inspection began at 14:15pm and ended at 17:00pm. The inspector reviewed compliance in the areas listed above. There were 39 children present and 9 staff. The inspector reviewed four children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 22.1-289.036-B-2
Providers must obtain a completed national criminal background check prior to the employee's first day of employment. Caregiver #1 and #2, who had been employed for one month, did not have a completed national criminal background check prior to their first day of employment.
Plan of Correction: Both employees received their criminal background check on day 3 of employment. Since their hire date was during staff work days, we allowed them to start once they sent their finger printing request in. They had their criminal background checks by the time children came back to school.
Standard 8VAC20-780-160-A
Each staff member shall submit documentation of a negative tuberculosis (TB) screening at the time of employment and the documentation shall have been completed within the last 30 calendar days of the date of employment.
The TB screening for Staff #1 was dated 17 months prior to the first day of employment. The TB screening for Staff #3 was dated five months prior to the first day of employment.
Plan of Correction: Both employees worked at a school immediately before they joined us and had TB tests done. We put their current TB tests on file and we were under the impression that we had 30 days to get an updated TB test. One was completed on 9/12 and the other completed on 9/18.
Standard 8VAC20-780-210-A
Program leaders shall be 18 years of age, have fulfilled a high school program completion or the equivalent and a minimum of 24 hours of training shall be received related to the care of children.
The record for Staff # 1 did not have supporting documentation of a high school program completion or the equivalent.
Plan of Correction: Our afternoon substitute when the lead teacher had a doctors appointment had 80 hours of training after her hire date. We did have her high school diploma in hand, but it wasn't filed at the time of inspection. Violation was corrected on the date of inspection.

Playground equipment with moving parts or climbing apparatus must have appropriate fall zones that encompass sufficient area to include the child's trajectory in the event of a fall.
The mulch in the fall zones on the large playground measured to be less than three inches in depth and some areas had bare dirt.
Plan of Correction: Mulch is already on site.

Sandboxes shall be covered when not in use.
The sandbox on the playground did not have a cover available when not in use.
Plan of Correction: Previously we were not required to add a cover. But covers have been purchased and are being used.
