Inspector Notes
An unannounced, on-site monitoring inspection was initiated on November 22, 2024, and completed on November 26, 2024, as a part of the licensure period. The on-site inspection began at 3:50 pm and ended at 5:00 pm. The inspector reviewed compliance in the areas listed above. There were 46 children present and six staff. The inspector reviewed five children's records and five staff records electronically on November 26, 2024.
This inspection included document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/06/2024. A POC submitted after this date will not appear on the public website.
Violations
4Child #1, who had been attending for two months, did not have documentation of an immunization record on file.
There was no physical examination in the record of child #1, who had been enrolled for two months.
Staff #4, employed for seven months, has not yet completed VDOE sponsored online orientation course.
Staff #1 obtained ten of the 16 required hours in 2023.
