Inspector Notes

A complaint inspection was completed on10/06/2022 about a complaint received in this office concerning allegations of being out of ratio and playground equipment having no resilient surfacing. A non-mandated unannounced monitoring inspection was also conducted on this day. The inspection started at 10:25am and ended at 1:00pm.

The evidence gathered during the investigation supported the allegations of non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-330-B
Based on observation, the center failed to where playground equipment is provided, meet the resilient surfacing requirement. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use.
Evidence:
1. On the asphalt playground, there was a saucer swing approximately 40 inches round with no resilient surfacing under it.
Plan of Correction: The swing will be removed as soon as possible.
Standard 8VAC20-780-350-B-1
Based on observation and interview, the center failed to maintain staff/child ratio in the infant room.
Evidence:
1. Upon entering the center at 10:25 a.m., two inspectors observed 12 infants with two staff (staff #1 and staff #2 supervising).
2. Staff #2 stated staff #3 was on a break. 3. The infant room was out of ratio for approximately 2-3 minutes.
Plan of Correction: 1. Try to keep keep staff and hire substitutes so that ratios will be met.
