Inspector Notes

An unannounced monitoring inspection was completed on 11/15/2022 to check compliance from the 10/06/2022 inspection. Twenty-one children were in care with 4 staff present at the beginning of the inspection. During the inspection, two additional staff arrived. The two inspectors reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 staff records and four child records were reviewed. The inspection started at 11:00am and concluded at 1:45pm.

There were two repeat violations, all other violations had been corrected.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation and interview, the center failed to maintain staff/child ratio in the infant/toddler room.
Evidence:
1. Upon entering the center at 11:00 am, two inspectors observed 13 infant and toddlers with three staff supervising (staff #1, #2 and #3). 2. Staff #1 stated staff #4 had an emergency and staff #5 was not there due to personal reasons. 3. Staff #4 stated one child had been out the previous day and was not expected today.
Plan of Correction: More staff will be hired including substitutes. Policies with staff will be reviewed and the ratio requirement.

Based on record review and interview, the center failed to have the required information for staff records.
Evidence:
1. Staff #1 was hired 11/15/22 and address/phone #, emergency contact name, address and phone #, and information about health problems that may interfere with fulfilling the job responsibilities, and two references were missing. 2. Staff #2 was hired 08/15/22 and is listed as program director and transcripts are provided, However, the copy is difficult to read.
Plan of Correction: Required staff information will be put in the file. A clearer copy of the transcript will be requested from the staff person.
