Inspector Notes

An unannounced inspection was initiated and completed on 1/8/2025 in response to a complaint received by the licensing office on 1/3/2025 relating to allegations of Administration, Staffing and Supervision, Programs, and Special Care Provisions and Emergencies. The inspector reviewed one child?s record on-site. This inspection included document review, interviews and observations.

The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Plan of Correction: The Director will develop a written plan for injury prevention.

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.

Child #1, who received a minor injury while in care at the center on 12/6/2024, did not have a written record of the incident.
Plan of Correction: The Director will ensure a written record of major and minor injuries is completed on the day of occurrence.
