During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 9/3/2025. The on-site inspection began at 12:15 pm and ended at 2:15 pm. The inspector reviewed compliance in the areas listed above. There were 41 children present and 8 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
At least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department, staff members shall obtain and submit the results of a follow-up tuberculosis screening.

Staff #4 did not have an updated tuberculosis screening in their record.
Plan of Correction: The Director will ensure an updated TB test is completed and placed in the staff's record.
Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

In the two year old and three year old classrooms, a cleaning solution containing bleach was not being kept in a locked place.
Plan of Correction: The Director will ensure hazardous materials, like cleaning supplies, are stored in a locked place.

The center shall meet the proof of child identity and age requirements as stated in ? 22.1-289.049 of the Code of Virginia.
