During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on and completed on 3/11/2026. The on-site inspection began at 9:00 am and ended at 10:30 am. The inspector reviewed compliance in the areas listed above. There were 40 children present and seven staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews and observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

In the preschool room there were spots of chipped paint.
Plan of Correction: The Director will report to the building's landlord about repairing the chipped paint in this room.

When the outdoor activity area is not adjacent to the center, there shall be at least two staff members on the outdoor activity area whenever one or more children are present.

Two separate classes went to the playground, that is not adjacent to the center, with only one staff member present.
Plan of Correction: The Director will update playground procedure to ensure two staff members are present.

Crib sheets shall be clean and washed daily.

Staff reported crib sheets were washed weekly.

There was no documentation showing a completed evacuation drill for December 2025, January 2026 and February 2026.
