Inspector Notes

An unannounced, onsite monitoring inspection was initiated on 08/27/2025 and completed on 08/28/2025. The on-site inspection began at 3:15 pm and ended at 4:25 pm. The inspector reviewed compliance in the areas listed above. There were 21 children present and 2 staff. The inspector reviewed 5 children?s records during the on-site inspection and 2 staff records during a virtual inspection on 08/28/2025.

The inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-510-P
When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization.

The authorization for Medication 1 has been expired for more than 2 months; the medication was still on-site.
Plan of Correction: Will request an updated authorization form and will send medication home until then.
Standard 8VAC20-780-530-C
There shall be at least two staff members who have a current certification in CPR and first aid present on the premises during the center?s hours of operation, on fieldtrips, and wherever children are in care.

Staff 1 was the only staff member on-site with a current certification in CPR and first aid.
Plan of Correction: A class is scheduled for 09/03/2025.

Staff records shall contain all of the required information.

The record for Staff 1 did not contain documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position.
