Inspector Notes

A complaint inspection was initiated on January 4, 2023 and completed on February 27, 2023 regarding concerns of physical plant, and special services. Interviews with staff and collaterals completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid.
