Inspector Notes

A subsidy health and safety (SHSI) inspection was completed. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. The children were observed during outside play time, getting ready for lunch, during lunch time, and getting ready for nap time. The inspection started at 11:00 am and concluded at 1:10 pm.

Information gathered during the inspection determined violations with applicable standards or law. Violations are identified on the violation notice.

Based on a review of records, the licensee did not ensure that staff who work directly with children shall annually complete the health and safety update course.
Evidence: The director confirmed that the records for three of the staff did not contain documentation of the department's health and safety update course:
1. Staff #3, VA preservice training was dated 8/10/2022
2. Staff #4, VA preservice training was dated 7/21/2022
3. Staff #5, VA preservice training was dated 8/19/2019
Plan of Correction: Ensure all staff complete the update training within 12 months each year using a specific list or spreadsheet for tracking.
