Providers must obtain a completed national criminal background check prior to the employee's first day of employment. Staff #2 did not have a new criminal background check on file. The staff member had been employed by the center previously but had been inactive for more than twelve months which requires a new one to be completed.

Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff #1 TB was dated six months prior to date of employment.

(Repeat Violation) Prepared infant formula shall be refrigerated, dated and labeled with the child's name. There were two bottles in the refrigerator in the infant room that were not dated.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #1, enrolled for two months, was missing the following required components: parent's work phone numbers, and a 2nd emergency contact.
