Inspector Notes
This inspection was conducted by licensing staff including documents review, interviews and a tour of the program. An initial inspection was initiated on September 13, 2022 and concluded on September 13,2022. The inspector reviewed compliance in the areas of administration, physical plant, programming, special care and emergencies and nutrition.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Standard 8VAC20-780-270-F
Based on observation and staff interviews, the licensee did not ensure to obtain all required equipment.
Evidence: There was not an operable nonpay telephone.
Plan of Correction: The center owner will ensure there is a phone by 9-14-22
Standard 8VAC20-780-330-B
Based on measurements, observation and staff interviews, the licensee did not ensure where playground equipment is provided, there shall be under equipment with moving parts or climbing apparatus enough resilient surfacing to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use. Falls zones shall not include barriers for resilient surfacing.
Evidence: There was not an adequate fall zone for a yellow climber as the fall zone available was measured 4?10 inches and minimum of 6 feet is required.
Plan of Correction: The center owner will have the landscape staff modify the fall zone on the backside of the climber to ensure there is at least 6 feet of resilient surfacing.
Standard 8VAC20-780-550-B
Based on documentation review and staff interviews, the licensee did not ensure the procedures for emergencies included all required items.
Evidence: The center owner confirmed the emergency procedures did not include staff training requirement and drill frequency for lock down procedures and shelter in place.
Plan of Correction: The owner will update the emergency evacuation policies and procedures by 9-14-2022.
Standard 8VAC20-780-550-I
Based on staff interviews, the licensee did not ensure to post emergency information.
Evidence: The numbers for 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center were not posted in a visible place at each telephone.
Plan of Correction: The center director will correct the issue by 9-14-2022
