Inspector Notes

An unannounced on-site non-mandated monitoring inspection was conducted on 01/10/2023. There were 43 children, ages 5 months-6 years, and 8 staff members present during the inspection. The inspector observed the following: free choice activities, morning snack, diapering, and large group time. A total of 2 children?s records and 2 staff records, were reviewed. The inspector discussed the following with staff: attendance requirements, online recording keeping and sanitizing solutions.

Based on review of two children's records, the
center failed to ensure that all children had a physical examination within 30 days of
enrollment.

Evidence:

1. The record for Child # 2, started on 09/12/2022, physical exam within 30 days of beginning attendance. The date on the physical exam was 11/01/2022.

Standard 8VAC20-780-440-G
Based on observation and discussion with the staff, the center failed to ensure that all children 12 months and younger were using a crib.

Evidence:
1. On the day of the inspection there were seven children 12 months and younger that were sleeping on an infant bed and not in the required crib per the standard.
2. Staff # 1 and # 3 confirmed that cribs were purchased but are not being used for nap time.

Based on review of the evacuation log and discussion with staff, the center failed to ensure that all monthly drills were conducted.
